Cancellation form

This is the model cancellation form in Schedule 4 to the Consumer Rights Act 2022. Use it for goods that have a 14-day right to cancel. Fresh food, food that expires quickly, and food made to order do not have that right. Faulty goods have separate remedies on the returns page.

When to use it

Complete and return this form only if you wish to cancel the contract. You can also cancel by a clear statement in an email or letter. Read the returns and cancellation page before sending food back. This page does not file the form. Print it, or copy it into your own email.

Complete this form only if you wish to cancel the contract. Then print it, or copy it into an email to hyvegrocery@gmail.com. Sending it from your own email is enough. This page does not submit the form for you.
